First Communion Form

1. A Baptism Certificate must be Presented to church with the registration;

2. Both candidate & immidiate family must participate in weekly Sunday Masses at Our Lady of Lebanon and in the parish Activities

3. Both Parents and candidates must volunteer at least 15 hours in church Activities and events (ex. Festival...)

4. Commitment and Guidelines Sheet must be signedbefore starting the classes.

5. Candidate must know the Our Father, Hail Mary, and the Glory before starting the classes
